Common causes
What tends to bring it on.
- Repetitive overhead activity, such as painting, swimming, throwing sports, or overhead trade work
- A sudden increase in load, such as starting a new gym program or taking on a big one-off job
- Age-related changes in the tendon, which become more common from the 40s onward
- A fall onto an outstretched arm or a direct knock to the shoulder
- Reduced strength or control around the shoulder blade, which changes how load passes through the tendons
- A previous shoulder injury, or ongoing strain from desk-based posture or manual work
Your AHPRA-registered physiotherapist starts by taking a history of how the pain began, what movements aggravate it, and how it's affecting your sleep and daily tasks. They then test your shoulder's strength, movement, and control, and screen for anything that needs closer medical attention. Most rotator cuff and shoulder pain can be assessed clinically, without a scan, unless there's been a significant injury or the findings suggest otherwise.
Treatment is generally built around graded exercise — loading the shoulder at a level it can currently manage and building this up over time, which is considered the main approach to longer-term improvement for most rotator cuff presentations. Hands-on techniques, such as soft-tissue work or joint mobilisation, may be used alongside this for short-term relief and to help you move more comfortably. Your physiotherapist also helps you adjust the activities that aggravate your shoulder, rather than stopping altogether.
For some people, particularly those returning to sport, manual work, or gym-based training, our exercise physiologists can work alongside physiotherapy to progress strength and load tolerance further. We make no promises about how quickly any individual shoulder will settle — tendons generally respond over weeks rather than days — but you'll have a clear plan and regular review along the way.
Your first appointment at our Goulburn clinic includes a full assessment of your shoulder, a working diagnosis, and a written treatment plan to take away, and in most cases you'll begin your first exercises or receive hands-on treatment in that same visit. You don't need a GP referral to book a physiotherapy appointment, though a referral is required if you want to use a Medicare Chronic Disease Management plan for rebated sessions. We typically review progress every few visits and adjust the plan from there — for most rotator cuff and shoulder pain, meaningful change is measured in weeks rather than days, with longer timeframes for larger tears or care after surgery.
