How it works

Allied-health care for older adults, in Goulburn.

Aged care at our clinic isn't a separate discipline — it's a population every clinician sees. Physios, OTs, podiatrists, speech pathologists, dietitians, exercise physiologists, and remedial therapists each carry older adults on their caseload, and the right match is the discipline that fits the problem. Falls and mobility sit with physio, EP, or hydrotherapy. Home set-up and daily tasks sit with OT. Foot care and diabetic feet sit with podiatry. Swallowing and communication sit with speech. Appetite, weight, and texture-modified diets sit with dietetics.

From referral to treatment

How aged-care visits run, step by step.

  1. Book in or refer

    Call us or book online. If your GP, geriatrician, or care manager is referring, they can send a referral by fax, secure messaging, email, or phone.

  2. We confirm the pathway

    Tell us how the visits are funded — Home Care Package, CHSP, Medicare CDM plan, DVA, or self-funded. Bring your care plan or care manager's contact details if there is one.

  3. First appointment

    We take the medical picture, ask about day-to-day life at home, do the clinical check that fits the case, and write a plan you can take home. Family or carers are welcome in the room.

  4. Treatment + reviews

    Sessions paced to the case, with the same clinician across every appointment. Where more than one discipline is involved, the team shares notes and writes one coordinated plan.

  5. Reports + paperwork

    We handle the documentation your funder needs — HCP / CHSP service-record entries, Medicare claim submission, DVA progress notes — so the medical plan keeps working without admin friction at your end.
